Step 1 β€” all age groups

The initial evaluation

Every patient β€” regardless of age β€” begins with the same first step: a comprehensive evaluation appointment. This involves a functional physiotherapy or feeding assessment followed by a medical consultation with Dr. Justin Roche. A recommended management plan is then discussed with you. No surgery takes place at this appointment.

The evaluation fee is the same for all age groups.

Children (6 months to 16 years): Functional assessment with a chartered physiotherapist, followed by medical consultation with Dr. Roche. Management plan discussed.

Adults (16+): Functional assessment with a chartered physiotherapist, followed by medical consultation with Dr. Roche. Management plan discussed.
A receipt will be provided. Depending on your health insurance policy, a portion of the medical consultation may be claimable.

Frenuloplasty under conscious sedation

Infants aged 12 weeks to 6 months are beyond the newborn period but too young for the approach used in older children. This age group requires functional frenuloplasty β€” a more comprehensive surgical procedure β€” performed under conscious sedation. This service is, to our knowledge, unique to the National Tongue Tie Centre in Ireland. Conscious sedation means your baby breathes independently throughout and is kept comfortable by local anaesthetic, avoiding the risks of general anaesthesia.

No upper age limit. We treat adults of all ages. Decades of compensatory patterns require a committed rehabilitation phase, but age itself is not a barrier. Many adult patients report that understanding and treating their tongue tie explains symptoms they have managed for their entire adult lives without a satisfactory explanation.
Health insurance

What your insurance may cover

We provide official receipts for all appointments and procedures. Coverage varies significantly between health insurance providers and between policy tiers β€” we recommend contacting your insurer directly to confirm what is covered under your specific plan before attending.

The majority of health insurance providers in Ireland reimburse for a lactation consultation (IBCLC). This includes the feeding assessment component of the infant evaluation appointment.

◐Medical consultation

Depending on your policy, a portion of the medical consultation with Dr. Roche may be claimable. A facility fee is charged for surgical procedures, meaning the full surgical cost is not covered by insurance.

◐Physiotherapy

Physiotherapy appointments may be partially covered depending on your policy. Check the annual physiotherapy benefit on your plan. Cover varies significantly between policy tiers.

A facility fee applies to all surgical procedures, meaning the full procedure cost is not covered by health insurance even where some coverage exists. Receipts provided for all components.

Fees β€” frequently asked questions

Why does the full pathway cost more than a single procedure elsewhere?
We do not offer a single-appointment procedure model. Our pathway includes pre-operative preparation, surgery, and post-operative rehabilitation as integrated components β€” because we believe this is what produces lasting clinical outcomes. A lower-cost single procedure may not include feeding support, physiotherapy, myofunctional therapy, wound review, or medical follow-up. The cost comparison is not equivalent when these elements are included versus excluded.
Am I committed to the full pathway when I book the evaluation?
No. The initial evaluation is a standalone appointment. We will assess your or your child's presentation honestly and advise on whether we believe treatment is indicated. If we do not believe it is clinically warranted, we will tell you that clearly. You are not committed to any further treatment by attending the evaluation, and the clinical advice and support from that appointment has value regardless of what you decide next.
Can I get a more exact cost before the evaluation?
The figures published here are "from" prices β€” they reflect the standard pathway. Individual clinical presentations may require more or fewer sessions, particularly in the pre- and post-operative therapy phases. We can give you a more precise estimate following the evaluation once we have assessed your individual clinical picture. If you would like to discuss cost in advance, please contact the clinic.
Is there a fee if we decide not to proceed with treatment after the evaluation?
The evaluation fee of €350 applies regardless of outcome. This covers the full assessment appointment β€” feeding or functional assessment plus medical consultation β€” and the clinical time involved in assessing your case and providing a management recommendation.
Do you offer a payment plan?
Yes. A payment plan can be arranged to spread the cost of the full pathway. Please contact the clinic directly to discuss the options available. We want the financial aspect to be as manageable as possible for families.
What documentation do you provide for insurance claims?
We provide official receipts for all appointments and procedures. We can provide itemised documentation and medical letters to support insurance claims where required. Please let us know in advance if you need specific documentation and we will do our best to facilitate this.
