## Who we are

The National Tongue Tie Centre (NTTC) was founded in 2007 by Dr. Justin Roche (MB ChB, FRCPCH, FRCPI, IBCLC), Consultant Paediatrician and Medical Director, and Kate Roche (BSc, MISCP, MCSP, IBCLC), Chartered Physiotherapist, Paediatric Feeding Therapist, and Clinical Director. Both co-founders hold International Board Certified Lactation Consultant (IBCLC) credentials. The integrated clinic opened at its current site in Clonmel on 5 January 2017.

## What we do

We provide specialist assessment and treatment for tongue tie (ankyloglossia) and related oral dysfunction across all age groups. Our clinical methodology is: Release Restrictions, Retrain Function, Relieve Tension.

Services include:
- CO2 laser frenectomy for infants 0 to 12 weeks
- Functional frenuloplasty under conscious sedation for infants and children aged 12 weeks to 12 years (the only such service in Ireland)
- Functional frenuloplasty for adults
- Pre- and post-procedure feeding therapy and myofunctional rehabilitation
- Paediatric physiotherapy and bodywork
- IBCLC lactation consultation

## Clinical philosophy

Tongue tie surgery alone is rarely sufficient for optimal outcomes. The NTTC's approach addresses:
1. The structural restriction (surgical release)
2. The functional compensatory patterns that developed around it (feeding therapy and myofunctional rehabilitation)
3. The fascial tension throughout the jaw, neck, and body associated with the restriction (physiotherapy and bodywork)

Pre-operative preparation is clinically essential, not optional. Every patient receives a written report at assessment before any decision to proceed with treatment is made.
